In this section of our website, you will find the following four forms:Medical Permit-to-Treat formBackground Check formDriver's Release formYouth Board application formThe forms are all available as pdfs - just click on the 'Download/View File' under the appropriate section to print out whichever form you need.There will NOT be any other postal mailings related to
these forms, so please let cindy@crossoflife.org know if you have any
questions about any of these attachments. Hard copies will always be available
at church too!

Driver's Release Form 2009-10
PARENTS - Fill out this form if you have not done so already, and would like to volunteer as a driver at any point during our program year.
Please click on the 'Download/View File' below to access a .pdf of our Driver's Release Form. Completed forms should be dropped off at church at least one week prior to the event in which you are participating. If you have a completed form on file with the church office, a new one does not need to be submitted.

Picture Scavenger Hunt 2009
2009 Picture Scavenger Hunt
Sunday September 27th
–6:00 –8:00 pm
Please join us for our annual Picture scavenger
hunt! We will be taking some very
memorable photos and enjoying an evening of laughter, creative poster making
and of course, snacks!!
Participants should arrive at Cross of Life at 6:00
ready to jump in a vehicle and get some great pics or video shorts.

Medical Permit-to-Treat form for 2009-10 Activities
One-time 2009-10 COL
Youth Events Medical Permit-to-Treat and Insurance Card Information Form
REQUIRED FOR EVERY SENIOR HIGH YOUTH
This
one-time form will cover all COL sponsored events outside of church for
the entire program year!
Click on the 'Download/View File' below to access your very own .pdf version of the form, then drop the completed form off at church on Cindy Boyd's desk. (There will be permission slips to complete for off-site events - please check the website as the event date nears.)

Background Check Form 2009-10

Youth Board Application Form 2009-2010
All interested 9th -12th grade youth are encouraged to fill out this application
if you are interested in joining the COL Youth Leadership Teams for this year.
If you have any questions regarding our COL Youth Board, please let Molly or one of the pastors know and they'll be glad to answer them for you. We will let our applicants know about
upcoming small group interviews for Youth Board in early September. We will
accept all applicants who are serious about helping our Youth Program develop
events, grow their faith and friendships, and serve their church! Just click on the 'Download/View File' to access a .pdf.
