2008-2009 Medical Treatment and Permission to Treat Form for 7th & 8th Graders
Please print out this form, fill it out, and return it to church (along with a copy of both sides of your medical insurance card). We'll keep it on file, and then your child can participate in Cross of Life activities. Please note that your child will NOT be permitted to participate in events outside the church without this signed form and a copy of your insurance card.
